What should we do when having tooth neck’s abrasion?

 Hi Tuấn Nam, as you described, your cases is the early presentation of abrasion of cervical neck of the tooth.

Abrasion of the neck of tooth is a common disease. This abrasion highly affect eating because  in people with abrasion, when eating hot, cold, sweet, piquant and sour will stimulate your tooth and make it painful and keen for you.

Cause of abrasion:

The following action can be the cause of abrasion

-          Traumatic occlusion

-          Improper brushing technique

-          Occupational ( habits such as holding pin in between teeth)

-          Tobacco chewing/ tobacco pipe

-          Vigorous use of tooth picks between adjacent teeth

-          Excessive mastication of coarse foods.

Clinical signs and symptoms of an abrasion:

They can be usually seen on both front and back teeth like incisor, premolars and molars. Based on cause and condition of abrasion, it can have different appearance.

-          The surface of lesion is extremely smooth and olished and it seldom has any plaque accumulation or caries activity in it.

-          The surrounding walls tend to make a V-shape, by meeting at an acute angle axially.

-          Peripheries of the lesion are angularly demarcated from adjacent tooth surface.

-          Probing or stimulating the lesion can elicit pain

-          Hypersensitive may be intermittent in character appearing and disappearing at occasional or frequently repeated periods.

Consequence of abrasion:

Abrasion might lead to periodontal diseases, pulp infection, may even lead to tooth lost, which greatly affect eating and aesthetic.

Treatment and prevention:

Brushing your teeth with soft brush, brush gently in circle motion. Using toothpaste with minimum or even without erosion factor.

Paste fluoride added varnishe to cover all of your teeth annually. Or using Toothpaste for sensitive tooth.

Changing your diet to lower colored food and addition chemicals to protect your tooth from staining and abrasion.

Avoid bad habits like chewing hard things, grinding, and treating malocclusions.

Annually visiting dentist every 6 months and keeping good oral hygiene.

If lesions is too deep and may cause disease to the pulp, you should have dentist put restoration to cover the lesion, restore shape and size of the teeth. Smoothen the surface around the lesions and adjacent teeth to limit chance of relapse.

If it affect your aesthetic (lower gingiva line, which affect your smile), gingiva surgery must be done.

For tooth with pulp badly affected, you may even need to undergone pulp therapy and might have to  do a big restoration with dental crown or verneer.
